Filling in an electronic application

Greetings. I pasted some links below to some images hosted by Photobucket. I
would ask that you copy them and paste them into your web browser (assuming
they aren't linked) so that you can see better what I'm talking about. My
problem is that I can't seem to fill out an electronic application without
ruining the format. By this, I mean that the spaces that are intended to be
typed into starting stretching when I try to type text into them. The lines
also appear to break. Any suggestions would be appreciated. Thank you.

The form illustrated was not intended to be filled out online; it was
intended to be printed out and filled in by hand or with a typewriter. You
can either fill in the form as intended or knock yourself out revising the
form to work properly.
