Faxes not sent when using a cover page

Hi,
I have a fax server installed and working on a Win2003 Server Standard
edition. The service worked fine for a while until I deployed it to a
large number of users (don't think it is the problem, but it is the only
event before the problem). When sending a fax with a COVER PAGE either
from a client station or directly from the server, the fax does not
appear in either the outbox or sent items folders and is not sent. I can
see an event that shows the fax was sent to the fax print device (on the
server) - No errors or messages are given. If I send the fax without a
cover page, it is sent.

I do not think it is a permissions issue since I am the Domain Admin and
have full Admin permissions at that server. This issue is driving me
nuts and I cannot find any reference for that problem.

Looks strange.
How are you sending the faxes?
Look at the event log for any error or warning events after the failure? I'm
not quite sure about the event that you are referring to in your post -
copy/paste the event text from event viewer. If you are using Send wizard,
can you check if the 'Preview' button in the last screen works?
Finally does the problem exist both for client-side (also known as personal
cover pages) & server-side cover pages?

3. The preview button works and displays properly.

4. I am using a server-side cover pages only, but testing a personal
cover page also gave the same results.
