"Enter password " window keeps popping up for no reason

The "Enter password" window keeps popping up and continues throughout my use
of Outlook. Everything is entered in it but when I click "sign in" it keeps
reappearing. My mail still comes through okay but it is annoying to have
this window constantly popping up without reason. I delete it but it comes
back later. How can I prevent this?
