My Network Password dialog box keeps popping up

I've had Outlook 2003 for several years. This started a few days ago, now
pops up so often I can't keep Outlook open. It prevents email from loading.
All info in the box is completed, i.e. server, user, and password (as ***).
Has a glitch entered my Outlook software? Help.
