email forms with a repeating question

I am creating an email form in outlook the asks 2 question 1 for an item name
and 1 for an amount. Is there a way after the 2 questions are filled in, it
would ask if you had another item and amount until you are done. exp if you
had 5 items and amounts it would repeat until you said you were done. Then
the email could be sent?

It might be possible to build such a form by adapting the technique demonstrated in the sample form at http://www.outlookcode.com/article.aspx?id=63, but it would take some effort. And, of course, it would only work if you could publish it to the Organizational Forms library.
