To set up an e-mail response form on my website I have been advised to use the GDFORM.PHP file. I have unable to find any information on any such program. Is anyone familiar with this file and its applications

You would have to ask whoever gave the the advice.

PHP is a server-side programming environment. Perhaps
someone has used it to write a form mailer, and called it
GDFORM.
