Email response form

I use FP 2002 with Office XP 2002. I want to add an email
response form to my website. My remote hosted server has
FrontPage extensions in place.

I am having trouble getting the Form commands to give me
an eMail response format. Has anyone done this
successfully? If so, please send advice.

When creating the form, you have a web open in FP, and not just the page,
correct, and when you publish the form the host, you are doing so via FP's
http mode, correct?

Under Form Properties, have you entered a email address that is within the
web site's domain and used the default email format?

Are you seen any error messages on the page or when hitting the submit
button, etc?
