e-mail response form

I have established an e-mail box and address hosted by my
web host. I have sent messages to this address and they
have been quickly forwarded to my personal inbox.

With help from this newsgroup I have put a response form
on the website. The submit button yields an automatic
response. But the message is not being forwarded although
I have followed the instructions on how to set up this
type of form. What could I be missing?

I had the very same problem! As it turned out my server didn't support the "forms" aspect of Frontpage extensions.
I had to log onto their site and use one of their "templates" for email forms and then modify it (best I could) to meet my needs. Tested it out and it worked fine.

Hope this helps, I'm still a rookie here. :)
