DX9 problems

I accidently started 2 instances of Everquest on my system, this caused an error. I shut everything down and restarted the computer and then tried going into EQ again, I get to a screen where DX9 is used and the screen goes blank. I then went in and ran DXDIAG, when the testing for DX9 occurs the screen goes blank. I reinstalled DX9, same thing. Reinstalled latest video card drivers same thing, reinstalled DX9 again, still the same thing. The file list in DXDIAG says everthing is find. DX 7 and DX8 test out just fine. Direct Draw works find. I have tried changing refresh rates to 60hz for all resolutions to see if maybe it was a refresh rate problem. Prior to this everthing was working just fine.

I think you're going to have to uninstall DX9 completely going back to dx8.
Then reinstall dx9. I have a dx9 uninstaller if you need it.
