Duplicate exchange accounts (Mailbox - username) in Outlook

Under "All Mail Accounts" in Outlook 2003, I have 2 listed “Mailbox –
Username†in which these accounts have the exact same name. When I click on
one
of them, it works just fine. When I click on the other, I get an error
message:

"unable to expand the folder. The set of folders could not be opened. This
error usually appears if the OST or PST file you are using is:
Unavailable
Protected with file permissions
On a share on a server and the network is down
Corrupt
To correct this problem, store the OST or PST file on the local computer.
You may also want to run the scanost.exe and scanpst.exe tools to verify
file integrity.

I have tried to right-click the duplicate that doesn't work to delete,
close, or remove it, but I get another error message that says "This group
of folder is associated with an email account. To remove it, on the tool
menu, click Email account, select view or change existing email account,
click next, select the account and then click remove." When I go to view or
change
existing exchange accounts, It only show one, not two. How do I get rid of the
duplicate accounts showing?

This account is actually for an Exchange Account. We changed the exchange
server recently and had to remove the existing account. However, something
must have went wrong and the bits and pieces of the previous exchange account
is retained. I have tried to repair, reinstall Microsoft Outlook, run scanost
and run scanpst but nothing seems to be able to eliminate the duplicate
Mailbox account.

Please help.

Here's the brief scenario

User with original profile (including Microsoft Outlook) - link to DomainA

We did a migration from DomainA to DomainB because we need to down the
original server which is hosting DomainA. This is to preserve whatever
profile that is existent for the user with minimal disruptions

User with original profile (including Microsoft Outlook) - is currently
linked to DomainB. Before DomainA is permanently down, we remove the exchange
account from the user (link to DomainA) and addback the migrated exchange
account in DomainB to the user's Outlook.

User ends up with two Mailbox(under all mail accounts) but only with the one
in DomainB being active. The other one could not be remove or purge. This
instance only happened to 1 of the 28 migrated system.

duplicate mailboxes or personal folders usually indicate a corrupt profile.

How do we eradicate this problem aside from creating a new profile from
scratch?

Kindly advise.
