Dial-up Connection Window won't close

I have had a problem with a dial-up connection window since I stopped
using Earthlink internet service in favor of DSL. A pop-up window
appears on my desktop with the following message: Select the service
you want to connect to, and enter your user name and password.

I have already tried the following fixes:
1.IE|Tools|Internet options|Connections|Never dial a connection
2.Control Panel|Administrative Tools|Services|etc. (sorry, I cannot
remember the rest of the path for this fix, but it failed also)

Any help much appreciated.

Go into 'Network Connections' and delete the connection to Earthlink.
Remove any Earthlink software from Add/Remove. Good Luck
