Custom Form

I would like to create a custom email message form where someone will fill in
text fields and check boxes in the body of the email and then return the
completed email back to me. Is this possible? If yes, could you point me in
the right direction where I could get some kind of tutorial on how I could
achieve this? I am using Outlook 2003.

Is it possible? Yes. Is it a good idea? Probably not. The only scenario in which that technique works is in an Exchange environment where everyone is using Outlook and you can publish to the Organizational Forms library.

Hi,

I have been searching help and google to find out more about custom forms. I
did read in the Help that you could use custom forms to sent emails to gather
informatin from recipients. I did actually create a custom form, created a
folder tried to publish to the folder but when I sent the form to myself, it
was just a default message form and my custom fields did not show. When I
published the form a dialog box asked me if I wanted to check something to
make the custom form available and I said yes. I pasted the fields in both
the compose and read page of the form but it didn't work. I just want ot send
an email message with some custom text fields that I want the recipient to
complete and return to me. Does anyone know what I am doing wrong.

The information in Help skips the prerequisites. Again: Do you know the recipient uses Outlook? Are all recipients inside your organization? Does your organization use Exchange as its mail server? Do you have permission to publish a form to the Organizational Forms library? If not, there's no point in pursuing a custom form solution.

The only way this can be made to work is by publishing the form to the Organizational Forms library or to each user's Personal Forms library. For basics on form construction, start at http://www.outlookcode.com/d/forms.htm
