Fillable forms in Outlook?

I'm wondering if it is possible to create a form that can be mailed out in the body of an email message, the receiver then completes the form by hitting Reply and filling out the from in the message body. Once complete, we would like to have a completed form but not as an attachment, We need to the form to be in the body of the email.

Any advice would be appreciated.
