use fillable forms in Outlook

I need to send a request for infomation to various point of contacts within
my organization. I want to use Outlook to send the request for information
via a fillable form, and use the calendar feature (or would the task feature
be a better choice?) to track who has/has not responded and to set a deadline
for reciept of the information. I could embed the fillable form as an object
but the recepient would have to double click on it for the form fields to be
activated. Also, I didn't have the opportunity to actually field test it to
see if I sent it the POC would actually recieve the form as fillable.

Do you use Exchange as your mail server? Can you get permission to publish forms to its Organizational Forms library? If not, Outlook doesn't have a workable solution for what you want to do.
