Try updating your modem with updated files from the manufacturer, if no joy:

Click Start > Run, type in: eventvwr.msc and press enter.
If a link to a possible solution is present in the error window, click it ot go
to the page.
If one isn't available, please post the error and description.

If you have another modem handy, swap it out for the one installed and see if
the issue is resolved.

The problem with eventid 1003 is it has many causes, although I suspect it's
either the modem drivers or software. Try uninstalling/reinstalling the modem
software and/or drivers to see if it resolves the issue.

Reboot to Safe Mode > Device Manager.
Remove any/all modem devices, close out and reboot.
Windows will redetect/reinstall the drivers.
Have the modem install disk on hand in case you are asked for it.

Start > Settings > Control Panel > Add/Remove programs.
Uninstall any modem software.
Reinstall the software.

If you wish to tshoot to find the exact cause, start here:
http://www.eventid.net/display.asp?eventid=1003&source=system+error

Check the link in Details and look for any error code parameter from event
viewer - 000000d1 is listed and more links to tshoot are available.
