Dan,

In case of a Common Dialog (same as Open/Save As in most Windows applications), this might help:

Right-click Taskbar, choose Properties, choose Start Menu tab

For XP Start Menu:
Click Customize, Advanced tab, and click Clear List button at the bottom

For Classic Start Menu:
Click Customize, and click the Clear button
Now the above-said items are cleared. Click OK, OK and close the dialog.

Note: This clears the recent documents history, the entries under Start-RUN dialog, open/save as (Common Dialog) boxes dropdown file lists and the typed URLs in the Internet Explorer Address bar.
