Can't boot into Windows

A while ago my system started getting disk read errors. I eventually
fixed this by repairing windows and using chkdsk from the recovery
console. Now I get the message to insert a boot disk and when I do I
get the error "Windows XP could not start because of a computer disk
hardware configuration problem. Could not read from selected boot disk.
Check boot path and disk hardware. Please check the Windows XP (TM)
documentation about hardware disk configuration and your hardware disk
configuration and your hardware reference manuals for additional
information. Boot Failed."
How do I fix this?

Try tapping the F8 key on starting computer,select safe mode,enter as
administrator,in xp chk event viewer for OS events.If that doesnt work,you
tried CHKDSK in recovery,did you type:CHKDSK C: /R The repair attempts
to fix bad sectors and goes thru the disk more thurough.

I did use CHKDSK C: /R. The boot disk was one made using the
insturctions on the microsoft website for Windows XP.

How do I fix this?

Get a diagnostic utility from the drive mftr. Download the file and make
a bootable floppy or cd with it. Boot with the media and do a thorough
test. If the drive has physical errors, replace it.

If the drive tests good, then the issue is probably with the
motherboard.

have you ever removed the HDD nad altered the position on ribbon cable or
jumper settings?

does HDD show up in computer bios?
As what? primary master?

Malke's suggestion to run diagnostic utility on HDD is a good idea, also.

Well, the hard drive was ghosted from my old hard drive professionally,
so I don't know the manufacturer. It shows up as primary master in the
BIOS.

what partitions are on the drive?
is there only one hard drive?
please post boot.ini file on hard drive and floppy disk??

There is only one partion on my only hard drive. The boot.ini file is:

start the recovery console.
at prompt>fixmbr say yes to create/fix mbr
at prompt>fixboot say yes to create/fix boot
