Hello Wil,
Now we have known this issue is related with certain third party software.
In my original post I have mentioned one method "Clean Boot". We could use
the following detailed steps to narrow down this issue further.
1. Click Start, and then click Run.
2. In the Open text box, type MSCONFIG then click OK.
3. On the System Configuration Utility window, click the Startup tab and
click the "Disable All" button to remove the checkmarks from each line in
the list.
4. Then, click the Services tab, add a checkmark to the "Hide All Microsoft
Services" item then click "Disable All".
Note: This disables all third party service items.
5. Click the OK button and restart your computer when you are prompted.
If the issue is resolved, it means the faulty item is among non-Microsoft
services or Startup items. Then you may start more services and utilities
each time, until we can isolate one utility or some utilities. Uninstall
this software.

In addition, another method is Add/Remove all the software in control
panel, until you could find *that* software.
You know this is a third party issue. We could only do this.
I hope the information proves helpful.
If you have any questions or concerns, please do not hesitate to let me
know.
Thanks and regards,
Alex Zhang

| OK - I (We) have narrowed it down. In safe mode the problem does not
occur. The double-pointed arrow shows up as it should.
So then I tried your second suggestion - Disable third-party browser
extensions.
Rebooted, and GUESS WHAT? - THAT WORKED. Now my double-pointed arrow shows
up again.
So we know that it's a THIRD PARTY EXTENSION, the question is which one?
Probably in the registry there's an entry for each third party extension.
I will have to - probably - disable each one at the time to figure it out.
Alex - What's the best way to do this?
Thank you again - you've been really helpful.
