Blank disk & installation

Now, I have a problem. I have two partitions: a Program/Boot partition (C)
(NTFS) around 10/20 gb) and a Data partition (D) (NTFS) (everything else).
I've just bought a blank HD. I want to install XP on the blank HD, but
during the setup process I can format only one partition (so in the end
after installation I'll have disk C, CD-ROM as D, second partition as E).
How can I partition and format the HD (in NTFS) BEFORE installing for the
first time the XP? (until now I simply installed twice XP, the first time to
partition the HD and the second time the real install)

Install Windows XP from booting from the CD and when you are asked about
partitioning just set the size of the boot/system/program partition to
whatever size you like.
Once installation complete you can then create additional partitions and
format them using the Disk Administration in Computer Management (in
Administrative Tools)

I know this, but this won't solve the problem that I want to have C and D
the HD and E the CD-ROM. I can swap the letters with the Disk
Administration, but then, for example, the Windows "knows" that he has been
installed from the D Drive and when he'll ask me the CD-ROM he will suggest
the D Drive.

You can change the CD ROM drive letter to whatever your wish.
If you take out your old HD and put in a new blank one the first partition
installed will be assigned driver letter C.
Once you have done that move the CD drive letter to E and create/partition
and format D

As regards your other issue around the source location that you insatlled
Windows XP from, just update the following registry key
HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows NT\CurrentVersion\SourcePath
