_vti_bin folder and FP forms

I am having trouble getting froms in htm pages to work. I created form pages
as ......./formname.htm. and the results files are in
the _private folder. My hoster also provided an interesting response: "It
appears that your form is making a reference to a page that either does not
exist or contains an error. The specific page is should be located within
the
_vit_bin. The exact link to this page is as follows.

http://website.com/_vti_bin/shtml.dll/formname.htm"

Any thoughts on this?

Does the file in the _private folder Guestbook_entries.csv exist?
Has your host enabled Email transport for the FP SE since you are sending mail too?
