Help! Basic Site w/ 1 form does not display or save results

No results displayed or save on the data entered.

I have reinstalled my IIS and Front Page 2003 on my XP Pro SP1 w/all updates.
Server extensions check out ok, test passed.

Created a "one page web site" using FP: http://localhost/mysite3
Added aa new form (using Forms Wizard) took the default settings for
contacts.
Save to the site. (Publish)

Next I open the site "http://localhost/mysite3/new_page_1.htm" in the browser.
Get the form opened.
Enter some data.
Click "Submit"
Next I get a Blank page at:
http://localhost/mysite3/_vti_bin/shtml.dll/new_page_1.htm

...No data is displayed, No Data saved in "_private/formrslt.htm" .

I check the send to path in the Forms properties and it points to
_private/formrslt.htm. (I've tried .txt as well)

I see the the file is created, but NO Data and No results shown.
I should note that data gets entered into the hidden folder
"_private\_vti_cnf".

Is there some setting somewhere.

Why is this not working?
What am I doing wrong?

Do I need ASP.NET?

Need some help to resolve this basic setup, Please!

Make sure you do not have the form to configure to send email while testing it on your local
machine.

The field is blank. No email address shown there.
It there another location for it?

Is there a setting for the Site in IIS that would prevent this from working?

Could the Server Extensions actually not be working even though it pass the
"Check Server Extensions" test?
