Word to PDF

I'm aware of all of the PDF Writers available to print to PDF, however I'm
looking for something to convert a 2003 Word Document into an
interactive/fillable PDF.

My word document uses the following controls from the control toolbox:

Text Box
Check Box
Option Button

Is there a software package that will convert a document with these controls
into an Interactive PDF Form that can be filled-out and printed w/ Acrobat
Reader?

Acrobat Pro comes with LiveCycle Developer that will let you do this but
it's not a simple click and you're done.
