Instead of going to Windows Update / Office Update Web
Site, is there any software can read which security
patches have been installed on my machine ? Does this
tool applicable to Windows XP as well ?

1) You can find the data in Control Panel; Add & Remove programs.

2) Belarc Adviser provides a list - and a lot of other data about your system.
Runs on Windows 95/98/Me, NT 4, Windows 2000, XP and Windows Server 2003.

(Not 100 percent sure it tells you about Office Updates.)
