windows explorer error

Ikeep recieving an error when trying to use ctrl c, ctrl b to copy paste in
the explorer
the error reported is: App Name: explorer. exe, App. Ver: 6.0 2800 1106.
Mode Name: Unknown. Mode Ver: 0.0.0.0. Offset: 04784590
 
sorry it is not ctrl b it is ctrl v, when I try to copy paste with ctrl c an
ctrl v, it happens some times not always.
