User name and password dialogue box

I never use this function but mistakenly asked to save a
user name and password (after a hard drive reformat) and
would:
1. like to delete the one saved dialogue
2. disble this dialogue box.

Can anyone help me with this.

Open Internet Explorer.
Click Tools
Click Internet Options
Click Content tab
Click AutoComplete button
Check/uncheck as desired.
