USB boot, stop 7B

I posted a question about stop 7B and USB boot a week ago. I've tried to
simplify things to eliminate unknowns. I have been using an IDE drive for
FBA for a couple of years without problems. On the same machine and with
the same TD configuration, I added the USB Boot 2.0 component, resolved
dependencies, and tried to execute FBA with a USB HDD and a 150Mbyte FAT32
active partition. After the gray on black progress bar completed, I got a
Stop 7B (0XF7C7A528, 0XC0000034, 0X00000000, 0X00000000).

The machine BIOS supports specifying the USB drive as the primary boot
device. Have I missed something that is needed to make USB boot work?
