A friend asked me the following - any suggestions appreciated (he's using
Outlook, not express)

"Often when we send emails with attachments, the message gets stuck in the
outbox and sends multiple times (until we physically delete it from the
outbox). It seems to happen when the attachment is > 1MB. Do you know what
causes this?"

The most frequent cause of this is two-fold: one, the server timeout value
on the Advanced tab of the account properties pages is set too low and two,
the sender is scanning messages with an antivirus program. The latter is
never needed.
