TOC Document Titles - 2 questions

(1) What "title" is used for documents (Word or pdf) in the TOC... the FP
page title, the FP file name, or the title of the page in the document's
properties?

(2) I've tried changing all 3 to eliminate the location showing, but some of
the the files still show. I did do an update all hyperlinks before
publishing.

If you right-click on the file and choose Properties, it's the Title you see
in the dialog box.

If you've already published these documents, changing the Title in your
local site is not going to force a republish of those documents. You will
need to open your remote site and change the Title there (then do a Tools >
Recalculate Hyperlinks to regernate the TOC.

Thank you for both answers. The thing is, changing the names via Properties
does sometimes work. That is why I was confused.
