Templates

I just created a template to be used at work. It has text boxes and drop
down boxes and will be used to evaluate staff competencies. After I
completed it (which took me forever) I realized that I may be in a heap of
trouble. Can I send a copy of it to another person via e-mail for them to
fill out, save, and send back to me without it loosing its code? I was so
excited to have completed one leg of this awesome task only to realize that
it may all be for naught. Can anyone shed light on this for me? Thanks.
 
If you have used form fields from the Forms toolbar, there is no "code"
involved. Save the document as a template and send it to others. They create
a new document based on the template.
