Table of Documents help

I have a report in which I need to add a TOC. I have
Executive Summary
1. Introduction
2. Evaluation Process
A. Assumptions
B. Findings and Explanation of Model Development
3. Refernces

I tried the standard way of inserting a TOC but it would not include
the Executive Summary or Assumptions and Findings

Can somebody help me out with this.

Did you apply the appropriate heading styles to the "Executive Summary,"
"Assumptions," and "Findings" headings? Or how are you creating your TOC?
