Stopping the "My Documents" list ?

How can I eliminate the "My Documents" item ?

I either need to remove it from the start menu or just stop it from
logging what files are worked on.

You should be able to do that via Group Policy. If this is a local computer
use gpedit.msc and go to user configuration/administrative templates/start
menu and taskbar and you will find options there to manage Documents. ---
