Sort by Icon - icon

When viewing Contacts by Categories there's a Sort by Icon column on the
far left - the default icon is a 'card'. I've redesigned the Contacts
form and now any new Contacts are showing up in this column with a
different icon - a note page with thumb tack. How do I get the icon back
to the default 'card' icon? (Outlook 2000 SR1)

On the (Properties) page of your custom form, in design mode, set the large and small icon to the default contact icon, which you'll find in Program Files\Microsoft Office\Forms\1033 (or something like that). Then increment the version number and republish the form with the same message class.
