Some captions don't refect in the table of figures

I've been using "insert captions" to make a table of figures. Some captions
do refect in the table as they are supposed to, but some don't. I tried
updating it, and making new captions and trying different ways, but I just
can't figure out why some do reflect and some don't...

Which version of Word? Note that if you add a caption to a floating object,
Word places the caption in a text box. And Word versions prior to Word 2007
cannot "see" captions in text boxes for the purpose of creating a table of
figures.

The workaround for Word 97-2003 is to place all figures "In line with text"
and also place the caption in line, as an ordinary text paragraph. You can
still position the figure and its caption by selecting them and adding a
frame around them. To add a frame to the selection, you can use the Insert
Frame button on the Forms toolbar.
