Rules and Alerts not always working

I have set up a rule for any message with [spam] in the topic to go to junk
E-mail folder. Some emails are doing so but some are not. Any idea what the
problem could be? None of the senders are white listed. I am using Outllok
2003.
 
Is this the first rule or are there other rules before it which do something
different?
