Revisions revert to an original form

Revisions, even inadvertent ones, continually revert to original form even
after document is "saved." IE: Paragraphs inadvertently are changed to
larger and underlined type. This is corrected and saved. Original form
reappears time and time again.

When you are tracking changes, you have to accept (or reject) the changes to
get rid of them completely. See
