Restoring from an old server to a new server - HELP

We move a server to new hardware. We get a stop error.

Then we do an in place repair/upgrade over the top and it goes though the
entire setup just fine.

After the last reboot after the setup we get

*** STOP: 0x0000007B (0xF681B848,0xC0000034,0x00000000,0x00000000)
INACCESSIBLE_BOOT_DEVICE

Why do we get this after a setup goes though and works fine?
