Reply's using an Outlook form

I have designed a simple Outlook 2002 form with a few
check boxes and some text fields. When I send the form
to a recipient it all looks fine. However, when they
reply, fill out the form and send it back to me, it does
not retain their information, the fields are blank.
 
Where is the form published? Are you working in an Exchange environment?
What steps are you following to send it to the recipient?
