Outlook Form Response - Option Buttons

I created a basic Evaluation Form in an email message, so I can send an email
ask them to select option buttons and add comments but once I send the form
to the recipient and they choose reply, they fill it out, all fields appear
to accept their selection but once it comes back to me there selections are
missing.

Any ideas on why the option button or the comment section (text box) are not
visible on return?

Did you bind all data-entry controls to Outlook properties, so that Outlook knows where to store the data?
