Create Fill-in Form - Outlook 2003

What I want to do is create a form with filling responses from an e-
mail recipient, who then fills out the form and replys back with the
form filled??

All my attempts to create fields and forms to do this have failed.
The form would be very simple (i.e.)

Name of Contact : _______________________

Contact Phone : _____________________

Send Reply (yes/no) ______ , etc.

Just fill-in Textboxes. Nothing fancy.

Everytime I create one, with designed Send and Reply , the returned
response is empty.

Can you help. Thanks

Outlook is not a suitable tool for this unless all recipients have Outlook, work in the same company as you, and use Exchange as the mail server. You must also have permission to publish to the Organizational Forms library on the server.
