Printer Set-Up- to Automatically Print "Page Address"

Can any one help? Thanks!

I would like to set up my printer preferences to
automatically print the "Document Address" [the entire
internet page address or the entire hard drive address for
the web pages or document being printed], as well as the
actual page number of each page in the printed output, and
the date/time of printing all shown in the header or the
footer of each page. E.g.:

[Document Name, if any] [Date/Time]

[TEXT BODY]

toolbar file button
page setup
click on [?] in upper right corner
click on header or footer box
make desired changes

in the yellow instruction window
do a alt+printscr and paste into wordpad document
