Printable documents

Hello I am new to FrontPage 2003. I have created a website and would like to
add documents to the website that clients print and fill out before they come
in for a consultation; specifically a health history questionnaire form and
some post care instructions. I have researched and found that it is not
always good web design to use PDF for viewing documents and it is ok for
printing documents.Plus it is only a couple of small documents.
Is there another way, using Frontpage 2003, to add this printable
form to the site besides using PDF and Adobe Acrobat. If I am completely
wrong in my decision not to use PDF ,what would be the best way to add PDF
documents and Adobe reader to the site.

Create a fillable pdf ( use Acrobat if you have it, or there are several
free pdf creators available).

Import it into FP, then create a link to the document.

When the viewer clicks the link it will open the pdf which he should be able
to fill out and print.

Hi,
if you need something filling out by the client PDF is the only way to go.
Most people will have the reader, you can provide a free download link for
anyone that doesn't
