Outlook Standard Contact Form - changed - how to make it The Stand

We need again your advise and support. Excuse my English writing. Thank you
for giving this.

We worked long hours to make a new Outlook form.
What is done #1:
1) In Outlook I opened a blank Contact form. Then under - Tools; Forms;
Design this Form - , I started to design this Contact form. This was most
time consuming.
2) I saved the form as: Patient Form 10 October 09h30.oft. When I want to
open this form than I get a message “The custom form could not be opened.
Outlook will use an Outlook form insteadâ€. Why I get this message, I do not
know. It might not even be important. At least I made a kind of back-up of my
Newly Designed Patient Form so that my work is not lost in case of a disc
problem.
3) Now it seems that I have to “publish this formâ€. So that this Newly
Designed Patient Form will appear every time a new contact is to be registerd.

Problem statement on #1:
1) I am unable, what ever I do, to allow this Newly Designed Patient Form to
be The Standard Contact form for my wife her Medical EMR DBase. If I can't
than all time is lost and the we are forced to buy an other software tool. I
don't want to do this. Please help.

Proposed Solution (if I may suggest):
1) Please send me the step by step instructions to make this - Newly
Designed Patient Form – to be The Standard Contact form for her Medical EMR
Dbase.
2) If you think it is better that I call you in order to give me support
over the telephone (on line). I am happy to invest into a long distance call,
from Frace, to obtain your support.
3) I am open to every other solution you suggest me to undertake to make
this Newly Designed Patient Form work.

Your analysis is correct: You must publish the form before it can be used as
the default form for a folder. Have you done that yet? If not, please see
