Contact Icon Chages When Form Created

Outlook 2003. We use a modified contact form as our default (a form I
created by modifying the standard contact form). Now, when new contacts are
added, they appear in the list view with a "postit" note icon rather than a
contact icon. The form comes up fine. It is just the wrong icon in the
list view. How can I change this back to a contact icon? Thanks for the
help.

In design mode set the large and small icons on the Properties tab. Make
sure to change the Version so it doesn't cause forms cache corruption and
publish the form.

Thanks Kevin. I figured it out. The (Properties) tab was not visible to me
until I maximized the design screen. I then had to find thye icons, which I
located at:

C:\Program Files\Microsoft Office\Office11\Forms\1033

The small icon is: Contacts.ico
The large icon is: ContactL.ico
