Outlook 2003 crashing on second opening

Hi there... I have the following problem...

Outlook 2003 will open perfectly the first time upon starting the computer, but after that, if the little Outlook Icon is sitting in the system tray and I try to start Outlook, it crashes.

When I try to double click the Outlook icon on the tray it does 'nothing'. If I kill the OUTLOOK.EXE process then start outlook it will work properly.

I'm currently using (Info from DXDiag) attached to this message

Please help...
