Numbering of Surveys

I need to individually number approximately 3,000 surveys, each consisting of
approximately 18 pages, with the survey number repeated on each page.
for example:
survey form #1 page 1, #1 page 2 .......; form #3000 Page 1, form #3000
pages 2 to 18.
Ideally this number will be recorded in the footer of the document and
automatically rollover rather than ahving to manually insert the number.
any assistance would be greatly appreciated.

For 3000 copies of an 18 page document, go to a commercial printer. They can
do the numbering for you, and the cost will be a fraction of what 54000
pages of printout would cost you.
