Number of Figures

I am using Word 2003. I am editing a very large document with many figures
and am numbering figures by chapter (e.g., 2.3, 2.4, etc.)

The Figure captions were being number correctly at Figure 4.12 but when I
inserted the caption for figure 4.13, the caption dialogue box showed the
Figure number to be 4.1. When I clicked ok, what was entered was Figure 0.1.
The chapter number was reset to 0 and the figure number was reset to 1 right
in the middle of the chapter for no apparent reason. I was not able to reset
caption number to display Figure 4.13.

How do I re-establish the correct sequence for both the chapter number and
figure number?

Is there a blank Heading 2 paragraph between the twelth and thirteenth
figure?

Another thing to consider: If you are using track changes, numbering
can be incorrect, until you've accepted (or rejected) changes.

I am not using tracked changes so that should not be an issue.

There are 3, 3rd level headings between figure 12 and 13. My Table number
had also started to do the same thing. Any ideas?

Thank you Stefan. After my reply to your post, I did some more investigating
and found one heading was incorrect. It was typed as if it was a level 3
heading but, in fact, it was a level 1 by mistake. Once I corrected that, my
number resumed correctly. Thanks for the clue.
