NTBackup restore problem

Enviro: W2kS up to date with SP's and patches. NTBackup with Backup
Assist. DAT24i. Doing nightly full backups which take two tapes.

Problem: When trying to restore a single file from tape NTBackup
disappears when it reaches the "Restore Progress" screen. No error
messages, no log files, no events in the event log. Files are not
restored.

Various facts:
1. RSM will appear confused after a restore attempt. Stop and restart
seems to cure it.
2. Backups are working flawlessly.
3. If I back up a file to a file the restoration of that file works.
IOW it only happens when trying to restore from tape, not file.

Things I have tried:
1. Different tapes.
2. Restart RSM.
3. Reboot server.
4. Replace NTBACKUP.EXE from know good server.
5. Deleting tape from RSM and re-cataloging
6. Restoring to original location as well as alternate location, even
different drive.

This is driving me nuts. I don't even know what to start looking at
because I have no errors or event IDs to reference. Any ideas here?

ntbackup.exe will always create a log file, even if it is a null
file. Look for backup??.log in c:\documents and settings\...

C:\Documents and Settings\Administrator\Local Settings\Application
Data\Microsoft\Windows NT\NTBackup\data

In here I find the backup report logs and there are logs for the
cataloging and backups but none for the attempted restores.
