Missing "lock" for Secure Connection

When I open the following URL in Internet Explorer (IE) v. 6 SP1 running
under NT 4 SP6a I get a secure connection, but there is no "lock" displayed
in the browser:

https://www23.addr.com/~alma/Order/order.mv

If I select File/Properites, the Connection description is: "SSL 3.0, RC4
with 128 bit encryption (High); RSA with 1024 bit exhange."

So, why doesn't the "lock" appear in the browser window?

Coincidentally, the lock does appear on IE 5.2.3 and Safari 1.2.4 running
under MacOS X 10.3.7.

Why doesn't the "lock" appear on IE 6 under Windows?

Just tried it in FireFox, and that warned me that the page contained some
unencrypted data, and the lock icon appears with a red slash through it. My
guess it's due to the stylesheet being importing from a normal http
connection. IE doesn't warn about this, but at least it gives an indication
that something isn't quite right by not showing the lock.
