minimized window won't come back up

Actually from what I can tell while using several different programs the
window suddenly disappears but is still running in the background and
showing up in the taskbar looking 'as if' it was minimized. It wasn't
minimized it just disappeared, Alt+Tab selecting the item in the task bar
that would normally bring it back up dosn't work.

Does anyone have any idea where the windows are going and how to get them
back or prevent it?
