Microsoft Office Outlook has encountered a user-defined breakpoint.

This morning Outlook 2003 (on Exchange 2003) was waiting for me with the
following pop-up message

Microsoft Office Outlook has encountered a user-defined breakpoint.

(A breakpoint in an application indicates a program error. After this
dialog is dismissed, the application will continue running, but it may
be in an unstable state.)


After I clicked on the Close button, "Outlook encountered an error and
needs to close. etc."

My question is: is this caused by Outlook itself, or could it have been
an add-in?
